Purchase your Milwaukee Ballet-made and approved masks online here.
Read about mask use from articles collected by our Health & Safety Task Force physician, Suzanne Martens, M.D.

Due to DropBox by midnight on June 29*:
– Medical Provider Statement of Good Health OR Parent’s Statement of Good Health
– Liability Release
– Copy of Health Insurance card, scanned front and back

*Each of the above forms are required and will replace any previously submitted forms. All forms must be submitted via DropBox.

Print the program supply list here. All students must follow the dress code and required face mask policy. Other items are highly recommended, but not required.
